Hi Folks

Let me clarify this for you. There's really (2) sets of procedures here:

1. You got the vaccinations in the DR

and

2. You get the vaccinations in the US

Now No. 1, if you got them in the DR and you have a copy of the vaccination form, just go to our download section and Dl the form I-693, fill out part 1 and print it, attach the copy of the vaccination form that you got from the DR and thats it, your done

Now No. 2
As TB said, if you are getting these done in the US, then same as above print out form I-693, fill in Part 1 and take that with you to the doctors office, they will fill out part 5 (which is form I-693A) seal all of this in an envelope for you and you include this with your AOS forms.

if thats the case then wats the point of saying u dont need to get the medical done here again.. to just send a copy of ur DS3025 and then fill out part 1, if the civil surgeon is going to fill it out .. he needs to do the medical all over again...

Hi TB

If you got the vaccinations in the DR, the form will be signed by the doctor, also remember and this is the really important part, all the originals, medical and vaccinations are given to the USCIS at the point of entry, all your doing here is showing them that you got this done when you file the AOS

ok... so to get everything straight, The Only thing K-1s have to do is fill out part 1 and sign it and send in a copy of ur DS3025... correct??? no civil surgeon has to do anything or be in a sealed envelope?? 

also.. do we need to print out the whole form I693 and send it in.. or just print the first page where the info goes??

The USCIS website has an entirely different form I-693, updated 4/02/08, and the site says they are NOT accepting any previous forms. (So the downloads section has to be updated I guess).

The new I-163 is five pages long, and actualy split into 'parts' as the directions describe.

*We have to fill out Part 1 (but don't sign ahead of time)

*Civil Surgeon will fill out Part 2 No. 5 (page 3),regardless of whether the shots were given in Sto Dgo or here.

*Then they have to filll out Part 5 (p.5) certification, then they have to follow the sealed envelope directions.

I actualy had to fax the Civil Surgeon nearest me the part of the directions which applied to this as they had no idea what I was talking about. However, the day before they quoted me a $250 cost for the physical, but once I faxed them the instructions and an explanatory memo, they offered to do this for my husband and daughter for just $100.

Correct, first page only, the only time the signature and sealed envelope come into play is if you get this done in the US

P.S. I will update our Dl section today with the new I-693, this change only occurred within the past week, that's how I found the new instructions (I-693 Instruction.pdf)

NO, this is the real deal, they DO NOT have to do the medical all over again, BUT some of them will try to make you believe they do. Go somewhere else if they tell you that and let them know why.

Is this true???  I am hearing 2 different stories here...some say we have to fill out the form even if we got shots in the DR, some say No....I am still confused...should I do it just to be on the safe side....or can somebody tell me that K-1's who got their shots in the DR just have to send in part 1 with the immunization record from the DR.

To be on the safe side SEND IT.. Maybe it's a new rule or the older k1s who got the shot in DR were lucky to get approved without sending the I-693 with AOS.
We only sent the DS 3025 and everyone got approved..

Evidently in the past K1/K2 folks who had their vacs done in the DR didn't have to go to a Civil Surgeon in the US, because the vac report you are given was apparently the same as the Vaccination Supplement to the I-693.That is where the confusion seems to be coming from.

However, the new form does not have a supplement, the vaccination page is actually page 3 of 5 pages, and the directions, on Page 4 under question 2, state that ALL K1/K2/K3/K4 who already had a medical exam overseas do not need a medical exam if AOS is within one year, BUT

"You will, however, be required to complete PART 1, information about you, and submit the vaccination section of Part 2 with you adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification."

Even filling out part one needs a Civil Surgeon, as it MUST be signed in front of the Civil Surgeon. But then the DOCTOR has to fill out the part about your Vacs - regardless of where you got them - and sign the letter/seal it/etc...

No, sorry you are confused. Understand that this new form is designed for a person that DID NOT get their vaccinations in the DR. If this has already been done, all you need is a copy of that report (which is available from the DR Clinic for $10US) and part 1 of the new form. The doctors signature has already been sent to the USCIS inside the manila envelope you gave at the POE.

As I said, if you got both the medical and the vaccinations in the DR, all you have to do, is get a copy of the vaccination report....period.. fill out part 1 of the new form, attach the vaccination report and send it in.

The new instructions on this form are designed fo people that are getting the vaccinations in the US, in that case, yes, the doctor will sign the new form, seal it in an envelope and give it back to you

OK, let's try to clear up a few things here.

First, you have always been required to fill out at least the vaccination supplement. A few people here have managed to get by with just the form from the Consulate but technically you are supposed to fill out the I-693 with ALL AOS applications. Some you have to fill out the whole thing and some you just need the I-693A which is the supplement. I would suggest making sire you read the directions carefully and fill out whatever it says to fill out. For each case there are specific instructions on whether you need the form or the supplement, but at any rate you probably shouldn't send just the DS-3025, you should accompany it with the proper form of the I-693.

This new form and instructions have just been published as of June 1, when the new vaccination requirements went into effect. Prior to June 1st, the only instructions available were on form I-485, where in the case of a K1, all that was needed was the receipt for the medical and the vaccination report.
For a K3, the medical and vaccination report was required.

However with these new instructions they have made things much easier, for both K1 and K3 visa applicants
