Ok we just found out that my wife is pregnant with twins and the
doctor states that the way it looks my wife will be on mandatory bed rest after 7 mnths. What are my chances of getting a visas for my inlaws for at least 3 mnths? I can provide a doctors note that states she will need help.

Tourist visas are very difficult to get in DR, but you can try.  They can show the CO a copy of the letter, but they are really looking to see what income they have, how much they have in the bank, do they own property -- essentially, what ties them to the country?  Do they have a reason to return?

It doesn't hurt to try but as LaBeba pointed out tourist visas are extremely hard to obtain.  Tourists applying for the visa are basically denied until you can prove to them (by "strong ties") that you they will return.
